Study Consent Form
What the study is about:
The purpose of this study is to learn about how people make choices.
What we will ask you to do:
If you agree to be in this study, you will be asked a series of questions. Some will be demographic; some will ask you to make choices between different options. Your results will be pooled with the results of others taking the survey. The survey should take about 20 minutes to complete.
Please note that no deception will be used in this study.
Risks and benefits:
I do not anticipate any risks to you participating in this study other than those encountered in day-to-day life.
Also, note that though measures have been taken to ensure privacy, information sent over the Internet is never completely secure.
As for benefits, your participation will increase our understanding of decision making, and you may learn something about yourselves.
Compensation:
Taking part is voluntary:
Taking part in this study is completely voluntary. You may skip any questions that you do not want to answer. If you decide not to take part or to skip some of the questions, it will not affect your current or future relationship with Cornell University. If you decide to take part, you are free to withdraw at any time. To skip a question, you will need to select the "No Answer" option from the drop-down box, or enter a zero in the text entry box.
Your answers will be confidential:
The records of this study will be kept private. In any sort of report we make public we will not include any information that will make it possible to identify you. Research records will be kept in a locked file; only the researchers will have access to the records.
